If the pet owner cannot be contacted and there are questions about how to groom the pet, the groomer will perform a routine bath and brush. If the pet is matted, the groomer may have to shave the pet for an additional charge. *

In order to keep our clinic free of external parasites, any pet with fleas, ticks or other ectoparasites will be treated at the owner’s expense. Crossroads Animal Hospital requires your pet to be current on Rabies, Distemper/Parvo and had Bordetella in the last six months in order to be groomed. *

I agree that my cat may need to be sedated for shaving. I realize there will be additional charges, and that although complications are rare, there are some risks involved (including cardiac problems and potential death), and release Crossroads Animal Hospital of all liability. *

I understand that grooming can be stressful to some pets, and I will inform Crossroads Animal Hospital if my pet(s) has any health conditions or any stress-related issues prior to grooming. Please note that your pet(s) will likely be groomed straight through and will need to stay with us for several hours at a minimum or for the day unless given a specific time. Pets that are sedated will need to be picked up at the end of the day so that we can ensure that they have safely recovered from sedation. The staff at C.A.H. are to use all reasonable precautions against illness, injury, or escape of my pet(s) and will not be liable or responsible in any matter whatsoever, under any circumstances, on account of the care, treatment, or safekeeping of my pet(s), as it is thoroughly understood that I assume all risks.
